Mental Health organization is excited about hiring an experienced Care Manager for community-based work. Must possess strong written and verbal communication skills. Have a history of working with underserved communities and be comfortable with mental health and/or physical health populations.

```Duties```
- Provide case management services to individuals and families in need
- Conduct assessments to determine client needs and develop individualized service plans
- Coordinate and monitor the delivery of services to clients
- Assist clients in accessing community resources and support services
- Advocate for clients' rights and ensure their needs are met
- Collaborate with other professionals, such as social workers and healthcare providers, to provide comprehensive care
- Maintain accurate and up-to-date case records

```Requirements```
- Bachelor's degree in social work, psychology, or a related field
- Experience in case management or a related field

Job Types: Full-time, Part-time

Pay: $60,000.00 - $62,000.00 per year
