Description

Under general supervision of the Transplant Social Work Manager, candidate is responsible for ongoing pre-transplant psychosocial assessment, waitlist management and post-transplant follow-up of Abdominal Transplant recipients as well as living donors. Candidates will assist with coverage of Cardiothoracic recipients as needed. Candidate will facilitate transplant and caregiver support groups and participate in various transplant committees.

Candidate should have a good working knowledge of local community resources. Must assist in coordination of discharge planning as needed and collaborate with transplant staff and social workers in other transplant programs. Position includes crisis intervention, mandated reporting, information and referral coordination of health and mental health services with community providers. The social worker is responsible for education and advocacy of patients, families and/or significant others. Candidate will participate in related department functions, such as community and program education. Performs all other duties as assigned.

Depending on the experience of the selected candidate, this position will be hired at a Clinical Social Worker II or Clinical Social Worker III level.

Minimum Qualifications

  • A Master's degree in social work from an accredited organization.
  • At least two years of post MSW experience. In order to be considered for the Clinical Social Worker III, must have at least three years of post MSW experience.
  • At least two (2) years working in an Acute Care Hospital and Health System as a medical social worker.
  • Experience working in both in an inpatient acute care hospital unit and outpatient specialty clinic setting.
  • Ability to work collaboratively as a member of a large multidisciplinary team.
  • Must have proven independent thinking, decision making, problem-solving and prioritizing skills.
  • Excellent skills to communicate and influence effectively with all levels of staff, physicians, patients, and external constituents, both verbally and in writing.
  • Excellent ability to multi-task with demanding schedules.

Preferred Qualifications

  • Experience working as transplant social worker.
  • Bilingual in English/Spanish.
  • Licensed Clinical Social Worker (LCSW) issued by the state of California. This is preferred for a Clinical Social Worker II, but required if being hired as a Clinical Social Worker III.

  • Must be able to work various hours and locations based on business needs.

Pay Transparency Act

Annual Full Pay Range: $85,232 - $112,293 (will be prorated if the appointment percentage is less than 100%)

Hourly Equivalent: $40.82 - $53.78

Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. The Hiring Pay Scale referenced in the job posting is the budgeted salary or hourly range that the University reasonably expects to pay for this position. The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and collective bargaining agreements (when applicable).
