POSITION SUMMARY: The Re-entry LPT will work in a residential setting that provides mental health and substance use treatment to individuals impacted by the criminal legal system. The therapist will collaborate with a multidisciplinary team to ensure comprehensive care that supports recovery and community reintegration occurs. This position offers a unique opportunity to make a meaningful impact on the lives of justice-involved individuals.
DUTIES AND RESPONSIBILITIES
· Applies principles of mental health in relationships with mentally ill or emotionally disturbed clients as a member of a therapeutic team at an outpatient facility.
· Observes, evaluates, records and reports to professional staff significant changes in the client's condition.
· Provides direct and indirect patient care including helping clients with personal hygiene such as bathing and personal grooming.
· Records measures of the client's physical condition such as pulse, temperature, respiration, weight, and body mass index.
· Prepares appropriate forms in connection with emergency calls, evaluation of the client's condition, and maintenance of the client's records.
· Assists professional staff members in providing and documenting occupational, recreational and vocational therapy programs related to the client's treatment plan.
· Administers, either orally or by injection, prescribed medications to clients in acute psychiatric settings
· Establish and maintain a therapeutic relationship with clients being served.
· Count and store medication in correlation to CCL. Document all medications on Centrally Stored and Destruction Records, create Medication Administration Records (MAR)as per each doctor’s order, count and control all medication brought into and out of the facility.
· Maintain a professional working relationship with contract agency staff, doctors, clinicians and caseworkers.
· Transcribes physician orders accurately and in a timely manner and provide staff with full understanding.
· Provide incoming shift staff with a detailed pass down relating to all medication issues.
· Document and notify the prescriber of any changes in behavior or condition relating to medication.
· Actively participate in multidisciplinary team meetings and treatment planning meetings as related to clients and medications.
· Demonstrates knowledge of crisis management and intervention
QUALIFICATIONS
· A valid, active, and unrestricted license as a Psychiatric Technician issued by the California Department of Consumer Affairs, Board of Vocational Nursing and Psychiatric Technicians.
· Previous experience with the population to be served and/or work in a similar program setting preferred.
· Three (3) years of experience in behavioral health, understanding of psychiatric illnesses and treatments, knowledge of psychotropic medications
· A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
· Excellent verbal and written communication skills.
· Excellent organizational skills and attention to detail.
· Must be at least 21 years of age.
· PPD test for tuberculosis and satisfactory results.
· Required Health Screening.
· CCL approved FBI and DOJ criminal background clearances.
· CPR/First Aid/BLS certification.
· Authorized to work in the U.S.
PHYSICAL REQUIREMENTS
· Ability to climb stairs multiple times a day and be able to lift and/or carry up to 25 pounds.
· Ability to sit, rise, kneel, squat, twist, pull bend multiple times a day.
· Ability to walk with items in hand, grasp and reach as needed.
· Ability to perform with repetitive maneuvers and manual dexterity.
· Ability to work in an office environment.
