SUMMARY

Housing for Health (HFH) is a program office within Community Programs, a division under the Los Angeles County Department of Health Services (DHS) for the County of Los Angeles.HFH was created to support people experiencing homelessness with complex clinical needs. We support people in obtaining housing, improving their health and thriving in their communities. HFH is a core component of Los Angeles County's effort to respond to the homeless emergency.Where appropriate to the job function, a hybrid work schedule may be available, with employees working both remotely and from the office, as needed.

The Mobile Clinic is a street medicine multidisciplinary teamthat will provide medical andbehavioral health care to people experiencing homelessness in the unsheltered setting.Directpatient care will be provided in a mobile clinic, encampments and on the streets. The behavioralhealth unit of the street medicine will include two clinical supervisors, Senior clinical socialworkers, substance abuse counselors, community health workers, and medical case workersThe patients that will be served include clients with complex medical mental health conditionsand those with high risk with complex behavioral health issues.

The Clinical Social Worker will be part of the behavioral health unit including the social work Supervisor, community health worker, and Substance abuse counselors.Under the direction of social work Supervisor, the senior clinical social worker will providebehavioral health services and case management services to patients who are eligible for enhanced care management to mobile clinic patients. The Clinical Social Worker coordinates social work services including consultation for Medical Case Workers, Community Health Workers, Substance Use Disorder Counselors, and other team members, for delivery of services related to mental health and social services linkages, community resources, and any presenting psychosocial issue.

ESSENTIAL FUNCTIONS

  • Assesses and treats the complicated psychosocial problems of homeless and formerly homeless individuals including but not limited to medical condition and/or functional status, untreated or under- treated mental health or substance abuse condition, economic instability, legal problems, and inadequate social supports, housing and transportation
  • Engages in case management services with patients of the mobile clinic to meet the agreed upon treatment goals.
  • Participates in improving chronic disease outcomes by providing primary and secondary preventive care services and self-management tailored to the patient's needs
  • Assists in increasing housing placement, retention, and maintain supportive relationships with the homeless and formerly homeless individuals during care provided in clinic and at the patient's home
  • Conducts comprehensive assessments and care planning of people experiencing homelessness and develops treatment plans that are sensitive to the patient's diverse needs and abilities
  • Provides clinical guidance and support to other Behavioral health and medical staff within the mobile clinic as needed and provide mentoring to new staff as needed.
  • Provides individual, group, and family mental health support and advanced level case management interventions, including but not limited to participating in treatment groups that support health promotion such as medication assisted treatment, group visits using motivational interviewing and a trauma informed approach
  • Use of Behavioral Health Techniques including, but not limited to, motivational interviewing, cognitive behavioral therapy, relapse prevention therapy, psychotherapeutic and/or systems interventions, and trauma-informed care, delivered in an individual and/or group setting
  • Provides consultation to the multidisciplinary teams to support management of chronic medical and behavioral condition and prevent acute care utilization through transitions of care interventions
  • Develops and implements methods for measuring effectiveness of social work practice and services in specialty areas, utilizing outcome evaluations to improve treatment services
  • Conducts comprehensive behavioral health assessments and care planning of mobile clinic patients and develops treatment plans that are sensitive to the patient's diverse needs and abilities
  • Participates in team huddles, case conferences, and multidisciplinary team meetings as needed
  • Participates in performance/quality improvement (PI/QI) activities and contributes to policy development as needed
  • Shares knowledge and effective practices with other Care Team members

JOB QUALIFICATIONS

  • Clinical social work in homeless health care experience
  • Experience/expertise in assessing and managing clients with dysfunctional SUD Experience/expertise in assessing and managing clients with severe persistent mental illness.
  • Experience/expertise in trauma-informed care and harm reduction principles
  • Demonstrated teaching and communication skills with clinical and non-clinical staff.
  • Experience providing clinical and administrative supervision to other behavioral health staff preferred.
  • Familiarity with DMH, DPH and community mental health and substance use programs and resources.

EDUCATION/EXPERIENCE

A master's degree in social work from a graduate school accredited by the Council on Social Work Education including supervised clinical social work field work experience - OR - A Master's Degree in Social Work from a graduate school accredited by the Council on Social Work Education and one year of experience in medical, psychiatric, family, or child welfare social work involving problems of medical, mental, or emotional adjustment.

CERTIFICATES/LICENSES/CLEARANCES

  • A valid, current license as a Licensed Clinical Social Worker issued by the California Board of Behavioral Sciences or registration in good standing as an Associate Clinical Social Worker with the California Board of Behavioral Sciences within six (6) months of employment. Associate Clinical Social Workers must remain in good standing with the California Board of Behavioral Sciences for the duration of the waivers permitted by law, not to exceed six years.
  • A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
  • Successful clearing through the Live Scan and the Health Clearance process with the County of Los Angeles.
