Job Title: Social Services
Reports To: Director of Social Services
Status: Non-Exempt
Position Summary:
Assist in the planning, development, and implementation of social services to ensure residents’ emotional and social needs are met, in compliance with federal, state, and facility regulations. Work under the guidance of the Director of Social Services and/or Administrator.
Essential Responsibilities:
- Assist with discharge planning by assessing residents’ physical, emotional, and social needs and coordinating appropriate post-acute services.
- Collaborate with residents, families, and healthcare teams to develop individualized care and discharge plans.
- Identify goals and action steps to address residents’ needs; coordinate with utilization management and insurance providers to ensure cost-effective care.
- Provide education on community resources and support options; coordinate referrals and service delivery.
- Attend care plan meetings, interdisciplinary rounds, and assist in resolving service gaps and discharge barriers.
- Conduct assessments for high-risk residents and provide psychosocial support and counseling.
- Maintain accurate and timely documentation and statistical data.
- Participate in performance improvement initiatives and community outreach as needed.
- Serve as liaison and advocate for residents/families, assisting with problem resolution and communication.
- Represent the facility professionally in external settings (e.g., court, panels, boards).
Qualifications:
- Bachelor’s degree in Social Work or related field required.
- Knowledge of social services regulations and best practices in long-term care.
- Strong communication, organization, and problem-solving skills.
- Ability to work independently and as part of a team.
