Provides assessment, consultation, and coordination for behavioral health patients through collaboration with care teams and external providers.
- Ensures appropriate patient placement, manages patient flow, and utilizes resources effectively, including self-referrals, agencies, hospitals, and internal departments like the ED.
- Offers 24/7 support delivering quality, team-oriented service, and facilitates communication to resolve issues aligned with organizational goals.
- Conducts assessments for self-referred and referred individuals, using medical and psychosocial knowledge to meet patient needs and address safety risks.
- Coordinates treatment recommendations with physicians and manages documentation in electronic records.
- Maintains current knowledge of civil commitments, manages involuntary admissions, and obtains insurance authorizations.
- Utilizes specialized software to monitor bed availability, patient flow, and data tracking for resource planning.
- Requires RN licensure, relevant mental health experience, de-escalation training, and strong communication skills; works weekends nights shift.
