Master’s Degree in Human Services + 1 year of experience OR Bachelor’s degree in Human Services +3 years experience.

Experience and/or interest in working with the homeless, at risk and individuals with substance use disorder and/or behavioral health diagnoses.

SCHEDULE: (non-exempt/salaried position)

Monday through Friday 8a-4p, with one 12p-8p shift per week

ABOUT THE ROLE: The Substance Use Disorder (SUD) Care Manager will work collaboratively with members of the Pittsburgh Mercy Family Health Center and external providers to deliver cohesive and patient-centered care. This position will provide clinically indicated case management support to patients of the Pittsburgh Mercy Family Health Center. The SUD care manager will engage with persons served, encourage independence, and maintain therapeutic boundaries, encouraging participation in medical care plan as well as be an integral member of the PMFHC multi-disciplinary care team. With a specialization in Substance Use Disorders, the SUD care manager will run and maintain the MAT clinic. Will support all aspects of the care plan as necessary and appropriate.
