Job Title: Licensed Master Social Worker (LMSW) – Weekend, Part-Time
Location: Bronx, NY 10467
Schedule: Weekends Only | 8:00 AM – 6:00 PM
Pay Rate: $55 – $58/hr
Type: Part-Time | 10-hour shifts

Position Overview:

We are seeking a compassionate and experienced Licensed Master Social Worker (LMSW) for a weekend part-time opportunity in the Bronx. The ideal candidate will have strong advocacy skills and experience coordinating care and support services across healthcare, social, and community systems.

Key Responsibilities:

  • Conduct comprehensive psychosocial assessments to identify patient needs, strengths, and support systems

  • Develop individualized care or discharge plans based on assessment findings

  • Coordinate referrals to community services including housing, transportation, financial aid, and food programs

  • Collaborate with interdisciplinary teams to ensure seamless continuity of care

  • Advocate on behalf of patients and families to secure necessary services and resources

  • Ensure follow-up on all referrals and service plans

Qualifications:

  • LMSW license in the State of New York (required)

  • Minimum 1 year of experience in a healthcare, community, or social services setting

  • Strong communication, organizational, and critical-thinking skills

  • Familiarity with available community resources and support networks

  • Weekend availability and commitment to high-quality patient advocacy
