Greenlife Healthcare Staffing is seeking a detail-oriented and driven Licensed Master Social Worker (LMSW) to join a high-impact team investigating complaints and surveying nursing home facilities across Central Islip, NY. What We’re Looking *

  • NYS Licensed Master Social Worker (LMSW)
  • Minimum 2 years of clinical experience in a medical setting (nursing home preferred)
  • Prior experience in nursing home surveillance is a plus
  • SMQT Certification preferred (or willingness to obtain within 12 months)
  • Strong written communication skills and the ability to travel locally for on-site investigations


What Youll *

  • Conduct onsite surveys and complaint investigations involving issues such as abuse, neglect, and care quality
  • Draft Statements of Deficiencies (SOD) using established regulatory principles
  • Evaluate Plans of Correction submitted by facilities
  • Enter critical findings into federal/state databases
  • Participate in state-level monitoring, especially during urgent or “Immediate Jeopardy” scenarios
  • Provide testimony in administrative hearings when needed
  • Attend regular team briefings and maintain compliance with DOH and IPRO protocols
