What We’re Looking *

NYS Licensed Master Social Worker (LMSW)

Minimum 2 years of clinical experience in a medical setting (nursing home preferred)

Prior experience in nursing home surveillance is a plus

SMQT Certification preferred (or willingness to obtain within 12 months)

Strong written communication skills and the ability to travel locally for on-site investigations

What Youll *

Conduct onsite surveys and complaint investigations involving issues such as abuse, neglect, and care quality

Draft Statements of Deficiencies (SOD) using established regulatory principles

Evaluate Plans of Correction submitted by facilities

Enter critical findings into federal/state databases

Participate in state-level monitoring, especially during urgent or “Immediate Jeopardy” scenarios

Provide testimony in administrative hearings when needed

Attend regular team briefings and maintain compliance with DOH and IPRO protocols
