What We’re Looking *
- NYS Licensed Master Social Worker (LMSW)
- Minimum 2 years of clinical experience in a medical setting (nursing home preferred)
- Prior experience in nursing home surveillance is a plus
- SMQT Certification preferred (or willingness to obtain within 12 months)
- Strong written communication skills and the ability to travel locally for on-site investigations
What Youll *
- Conduct onsite surveys and complaint investigations involving issues such as abuse, neglect, and care quality
- Draft Statements of Deficiencies (SOD) using established regulatory principles
- Evaluate Plans of Correction submitted by facilities
- Enter critical findings into federal/state databases
- Participate in state-level monitoring, especially during urgent or “Immediate Jeopardy” scenarios
- Provide testimony in administrative hearings when needed
- Attend regular team briefings and maintain compliance with DOH and IPRO protocols
