Position Overview

We are currently seeking a Licensed Master Social Worker to serve as a Nursing Home Surveyor / Complaint Investigator with a review company located in New Rochelle, NY. This full-time role involves conducting investigations and surveillance in long-term care facilities to ensure regulatory compliance.


Qualifications

  • Active NYS Licensed Master Social Worker (LMSW)
  • Minimum 2 years of clinical experience in a medical facility, preferably a nursing home
  • Experience with nursing home surveillance activities (preferred)
  • SMQT Certification (preferred, must be obtained within 12 months if not already held)
  • Valid driver’s license and ability to travel to on-site locations
  • Must be comfortable with documentation, reporting, and state/federal compliance procedures

Schedule

  • Full-Time
  • Monday – Friday
  • Flexible hours between 7:00 AM – 7:00 PM


Responsibilities

  • Conduct surveys and complaint investigations, including abuse and neglect cases, in nursing homes
  • Document findings and draft Statements of Deficiencies (SOD) in compliance with Principles of Documentation
  • Evaluate compliance with Plans of Correction
  • Input required information into state and federal reporting databases
  • Participate in immediate jeopardy monitoring situations
  • Attend meetings and testify at administrative hearings when necessary
  • Adhere to DOH and IPRO schedules, standards, and protocols
  • Travel to assigned facilities in accordance with the work schedule
