• Conduct psychosocial assessments to support discharge planning.
• Collaborate with case managers and healthcare teams to develop safe discharge plans.
• Connect patients with community resources, financial aid, or housing support.
• Educate patients and families on care options and post-discharge services.
• Address barriers to care such as lack of support, transportation, or insurance issues.
• Document all assessments, interventions, and outcomes accurately in the medical record.

Minimum **

• Must have an active LMSW NY State.
• At least one year of recent experience is required.
• A master is required
