Job Summary:

Hospice is seeking a compassionate and experienced Spiritual Counselor to provide spiritual and emotional support to patients, families, and caregivers in alignment with their beliefs, values, and end-of-life goals. This dual-role position also oversees the bereavement program, offering grief support and coordinating services for families following a patient's passing.

Responsibilities:

Spiritual Counselor Duties:

  • Provide spiritual support and counseling to hospice patients and families, respecting diverse beliefs and faith backgrounds.
  • Assist patients and families in coping with the emotional and spiritual challenges of terminal illness and dying.
  • Collaborate with the interdisciplinary team to ensure spiritual needs are incorporated into the plan of care.
  • Conduct or coordinate spiritual rituals, prayer, or religious ceremonies when requested.
  • Document visits and services provided in the clinical record in a timely manner.

Bereavement Coordinator Duties:

  • Develop and maintain the agency’s bereavement program in compliance with Medicare Conditions of Participation.
  • Conduct bereavement assessments for family members and caregivers after a patient’s death.
  • Provide one-on-one grief counseling, support groups, and educational materials.
  • Coordinate bereavement services for at least 13 months following the patient’s death.
  • Maintain bereavement records and track services offered and delivered.
  • Assist in planning and facilitating memorial services or remembrance events.

Qualifications:

  • A bachelor's Degree is Preferred.
  • Ordained clergy, certified chaplain, or spiritual care provider with experience in hospice, palliative care, or grief support.
  • Minimum of 6 months of experience in spiritual counseling or bereavement services, preferably in a hospice or healthcare setting.
  • Strong communication, counseling, and organizational skills.
  • Valid driver’s license, auto insurance, and reliable transportation.
  • Knowledge of diverse cultural and religious practices related to death and grief.

Compensation:

  • Pay Rate: $130 per completed, documented spiritual care visit.
  • Bereavement program coordination may be compensated separately depending on the caseload and scope.

Pay: $135.00 per hour

Expected hours: 135 per week
