Job Title: Social Worker
Location: Seagate Rehabilitation and Nursing Center, Brooklyn, NY
Shift: Full Time, 9 AM - 5 PM
Salary: $60,000 per year

Responsibilities:

• Interview residents and families to obtain social history.
• Involve residents and families in planning social service programs when possible.
• Assist in providing solutions for social and practical environmental problems, including seeking financial assistance, discharge planning (collaborating with community agencies), and referrals to other specialized services.
• Collaborate with administration, medical and nursing staff, as well as other departments in planning social service programs and activities.
• Participate in Facility Surveys (inspections) made by authorized government agencies, as necessary.
• Interpret social, psychological, and emotional needs of the resident and family to the medical team, including attending physicians and other resident care team members.

Requirements:

• Minimum 1 year of experience working in a Skilled Nursing Facility.
