Join our compassionate and dedicated team and make a meaningful difference in the lives of our members.RiverSpring Health Plans serves over 18,000 frail and disabled individuals, empowering them to live independently in their homes for as long as possible.As a Nurse Care Manager, you will play a vital role in care coordination, including in-home assessments, planning, facilitation, advocacy, and authorization of covered services to meet members' health needs while promoting quality, cost-effective outcomes.Key Responsibilities:Collaborate with members and their families to maintain the most independent living situation possible.Ensure consistent care across the healthcare continuum by assessing and closely monitoring members' needs and status.Authorize covered services and coordinate care across different payers.Communicate and collaborate with members, family members, caregivers, primary care practitioners, and the interdisciplinary team.Qualifications:Current NYS RN License and registrationBachelor of Science in Nursing (BSN)Minimum of 3 years of RN experienceClinical expertise in geriatrics preferred; managed long-term care experience is also a plus.Bilingual SpanishCompensation:Annual salary: $105,000 - $115,000Eligible for a 20,000 sign-on bonus ($5,000 payable every six months, less taxes)Benefits include health insurance, retirement plans, paid time off, and professional development opportunitiesIf you're ready to make a difference and support older adults in their journey to live independently, we would love to hear from you!Equal Opportunity Employer (EOE)
