We are looking for a PRN Social Worker to join our team!

Responsibilities.

  • Perform Psychosocial assessments, including social and emotional factors related to patient and family.
  • Provide individual and family education/counseling in regards to terminal illness.
  • Creates individualized care planning, evaluation and revision as appropriate to patient/family needs.
  • Assist patient/Family with community resources.
  • Participates in Interdisciplinary Care Meetings.
  • Support in the QAPI Initiatives and Activities of the Agency
  • Participate in the Educational Activities to the Community about Hospice Program

Skills

  • Positive Attitude and Team Player.
  • Strong communication and Problem Solving skills.
  • Sensitivity to the impact of end of life issues.
  • Knowledge of resources available in the community to assist Patient/family.
  • Bilingual (English and Spanish)preferred
  • Technology Proficient

Job Type: Part-time
