Job Title: Care Manager RN

Job Summary:

The Care Manager RN is responsible for coordinating, overseeing, and implementing healthcare services for patients. This includes assessing patient needs, developing care plans, collaborating with healthcare providers, and advocating for patients. The Care Manager RN works to ensure the highest quality of care for patients and promotes positive health outcomes.

Responsibilities and Duties:

1. Assessing patient needs and developing individualized care plans

2. Coordinating healthcare services and resources for patients

3. Collaborating with healthcare providers to ensure continuity of care

4. Monitoring patient progress and adjusting care plans as needed

5. Advocating for patients and ensuring their needs are met

6. Providing education and support to patients and their families

7. Documenting patient interactions and treatment plans

8. Participating in interdisciplinary team meetings

9. Adhering to all healthcare regulations and policies

10. Maintaining confidentiality of patient information

Qualifications and Skills:

1. Registered Nurse (RN) license

2. Bachelor's degree in Nursing (BSN) preferred

3. Minimum of 2 years of clinical experience

4. Strong communication and interpersonal skills

5. Ability to work independently and as part of a team

6. Excellent organizational and time management skills

7. Knowledge of healthcare systems and resources

8. Ability to assess patient needs and develop care plans

9. Proficiency in medical documentation and record-keeping
