About Job
- Assess patient and family needs related to coping, mental health, financial resources, and caregiver support.
- Provide individual and family counseling, crisis intervention, and bereavement support.
- Collaborate with the Interdisciplinary Group (IDG) to develop and manage patient care plans.
- Assist with financial resources, Medicare/Medicaid, and long-term care planning.
- Maintain accurate electronic medical records (EMR) and participate in IDG meetings.
- Support patients and families with advance directives and memorial planning.
- Educate the community on hospice care.
- Participate in an on-call rotation.
- Education & Experience: Minimum one year of social work experience in a healthcare setting (internships may qualify).
- Licensure: Must hold or be in the process of obtaining one of the following Florida licenses within 90 days of hire:
- Licensed Clinical Social Worker (LCSW)
- Licensed Mental Health Counselor (LMHC)
- Licensed Marriage and Family Therapist (LMFT)
- Other Requirements:
- Valid Florida driver’s license and reliable transportation for field-based roles.
- Proof of valid auto insurance.
