Purpose of this position

The purpose of this position is to supervise, provide and coordinate nursing care in compliance with facility policies and procedures and to assess residents’ care needs and take appropriate action.

Qualifications

Education: High School Graduate or GED

Licensure: None required

Experience: One year experience in long term care as a caregiver.

Delegation of authority

Authority is delegated to the individual in this position to:

· Take appropriate action to assure adequate staffing to provide resident care.

· Supervise and assess resident care and take appropriate action.

· Implement facility procedures to provide resident care.

This position reports to: The Executive Director

Duties:

1. Follow established standards of nursing practices and implement facility policies and procedures.

2. Interpret existing policies and procedures to nursing assistants, medication aides, residents, families and physicians.

3. Supervise and evaluate all direct resident care and initiate corrective action as necessary.

4. Receive direction from the executive director and implement appropriate follow-up action as necessary.

5. Provide direction to care staff, including sufficient information for follow-up action as necessary.

6. Take appropriate action to assure adequate staffing to meet residents’ needs.

7. Conduct resident rounds multiple times daily, reporting problems to the physician and director and initiating corrective action.

8. Provide supervision to nursing assistant providing direct resident care.

9. Attend staff meetings, interdisciplinary team meetings, rehabilitation meetings, in-service classes and committee meetings as assigned.

10. Follow staffing guidelines and make nursing assignments according to qualifications of staff and characteristics of the resident population.

11. Assume responsibility for compliance with federal, state and local regulations within the assigned unit.

12. Counsel nursing assistants and medication aides and recommend disciplinary action to the director, when necessary.

13. Participate in the employee evaluation process by providing information on the performance of nursing assistants.

14. Administer and document direct resident care, medications and treatments per physicians’ orders and accurately record all care provided.

15. Provide instruction for appropriate nursing care to care staff.

16. Perform and document pre-admission assessment, initial and subsequent service plans, and other documentation of residents as assigned.

17. Promptly report changes in residents’ conditions to the physician, director, and responsible party, and take follow-up action as necessary.

18. Follow facility admission procedures and communicate resident admission to other departments, physicians and ancillary service providers.

19. Follow facility discharge /transfer procedure and coordinate discharge/transfer with other departments, physicians and ancillary service providers.

20. Complete documentation related to the resident’s discharge/transfer.

21. Count narcotics, check emergency kit and exchange keys if administering medications.

22. Supervise meal time and check meal trays and nourishments to ensure proper diets are served and residents are assisted as necessary.

23. Coordinate outside resources for residents such as home health, medical equipment, transportation, and medical appointments.

24. Order, receive and store medications appropriately.

25. Review residents’ medical records and physician orders and ensure appropriate follow through on orders, care, or treatments.

26. Complete incident or accident reports for residents, staff, and visitors when occur. Follows up on incident with investigation and corrective action.

27. Notify physician, when necessary, of changes in residents’ conditions, weight, medication errors, medication not available, abnormal laboratory/X-Ray results and any other unusual observations.

28. Document resident progress notes as required. Document daily notes on unstable residents and recently admitted residents.

29. Consistently work cooperatively with residents, nursing staff, physicians, families, consultant personnel, community agencies and ancillary service providers.

30. Observe all facility policies and procedures.

31. Come to work as scheduled and consistently demonstrate dependability and punctuality. Consistently present a professional appearance.

32. Perform other related duties as directed by his/her supervisor.

Job Type: Full-time

Pay: $37,440.00 - $41,600.00 per year
