ProSource Talent is seeking a dedicated Medical Care Coordinator to provide direct case management for patients in our Primary Care Clinic. This role ensures patients maintain access to medical care and receive the supportive services needed to improve health outcomes.
Key Responsibilities:
- Deliver proactive case management services including intake, assessments, and individualized care planning to ensure consistent access to medical care.
- Maintain regular communication with primary care providers to support patient progress and treatment goals.
- Coordinate referrals and link patients to specialty care, entitlements, behavioral health services, ADAP, and other supportive programs.
- Obtain prior authorizations for medications or services not covered by insurance plans.
- Accurately document all services in both electronic and paper records in compliance with agency and funder requirements.
- Support outreach and engagement efforts to connect Asian and Pacific Islander individuals living with HIV/AIDS to care and services.
- Collaborate with multidisciplinary clinical and client services teams on continuous quality improvement initiatives.
- Participate in additional activities as assigned.
Qualifications:
- Bachelor’s degree in social science, psychology, social work, health, or a related field — or equivalent experience — required.
- Licensed Practical Nurse (LPN) with at least 1 year of relevant experience may also qualify.
- Associate degree (AA/AS) in a health or human services field plus 3 years of related experience may be considered equivalent to 1 year of a bachelor’s degree.
- Candidates with a non-related bachelor’s degree and 5 years of relevant, post-degree experience (including customer service) may also be considered.
- At least 1 year of case management experience preferred.
- Experience working with at least one of the following populations is required:
- People living with HIV/AIDS
- Asian and Pacific Islander communities
- Immigrants
- LGBTQ+ individuals
- Families with children
Job Type: Full-time
Pay: $25.00 - $29.00 per hour
Expected hours: 40 per week
