Qualifications:

• Must have either BSW or MSW
• Must have previous experience as a Social Worker
• Must have long-term care experience/ nursing home experience
• Must be a self-starter and energetic
• Strong written and verbal communication skills

Social Worker Job Description:

• Understand and meet all government requirements for social service documentation.
• Assist new residents and their family in the initial adjustment phase, as well as in the ongoing transition to institutional living.
• Create personalized care plans that address both immediate and long-term needs.
• Provide information to the resident, and/or family member, on residents' rights and responsibilities.
• Provide counseling and emotional support to help residents and their families navigate the challenges of long-term care.
• Identify community changes and opportunities such as legislation, regulations and programs that affect nursing home residents.
• Participate in quality assurance interdisciplinary team meetings.
