Description
Under the general administration of the Medical Director of the General Internal Medicine Clinic, the incumbent in this position is responsible for providing education and counseling to patients in the General Internal Medicine ambulatory clinics.

SERVICE 100%
- Develop and deliver a program of counseling and education for patients referred by Providers of the General Internal Medicine Division. B. Provide interventions to support patients and their families, during the recovery from and/or adjustment to acute or chronic medical conditions. C. Participate in team meetings to discuss and develop programs and treatment plans. D. Assist in identifying patients, social determinants of health and connect/refer to the appropriate available community resource. E. Function as an integral part of the multidisciplinary healthcare team. F. Maintain and communicate to multidisciplinary healthcare team members a list of available community resources. G. Provide support to patients of the Medication Assisted Treatment Clinic. H. Other duties as assigned.
MINIMUM QUALIFICATIONS
- Masters of Social Work from an accredited college or university. 2. Current valid license by the State of Illinois as a licensed clinical social worker. 3. Sensitive to the needs of underrepresented minority populations
