Intake Specialist Position - Broad Street Home Care
BSHC is seeking a full-time Advocate Intake Specialist to join our growing team!

At Broad Street, we don't look to drive our clients' decisions, but to provide comprehensive support to live the life they choose. We are a Concierge Health Service which is private pay, we don't have the administrative burden of managing Insurance and the related coding, so we are free to focus on Care.
Our Intake team are consultants first who listen to learn our clients' needs to help our patients navigate their best home care decisions. Intake Specialists are strategic advisors to our clients, helping to provide recommendations to create the best patient outcome. We not only support our patients but our teammates as well, allowing for work-life balance and flexibility.
Work Location: Remote/Hybrid - This position will require travel to client/patient residences, healthcare facilities, and on occasion a Branch Office, but it is otherwise remote.
Schedule: Full-time, with one day off per week. The role involves being on call every other weekend, with shift trades coordinated with a colleague partner. As part of a team, you will work together to establish the on-call schedule and respond to emergency calls.

What You'll Do
- Client Engagement & Needs Assessment: Conduct thorough conversations with potential clients to understand their needs, preferences, and concerns, ensuring a comprehensive understanding of their situation.
- Service Education & Guidance: Provide detailed information about our home care services, guiding clients through the options available to them and helping them make informed decisions.
- Coordination & Referral Management: Collaborate with HR to facilitate assessments and referrals, ensuring clients are matched with appropriate services and caregivers.
- Documentation & Record Keeping: Maintain accurate and confidential client records, ensuring compliance with company policies and regulations, while documenting all interactions and assessments.
- Follow-Up & Support: Engage in follow-up communications with potential clients to address any additional questions or concerns, ensuring a supportive experience throughout the process.
- Brand Representation: Actively represent our company's values and commitment to quality care, promoting a positive image and fostering trust with potential clients and their families.
- Resource Connection: Serve as a liaison between potential clients and internal teams, connecting clients with resources and support that meet their specific needs.
- Communication Skills: Exceptional written and verbal communication skills, with an ability to convey complex information clearly and empathetically.
- Proactive Problem-Solving: Ability to work independently, identify needs, and implement effective solutions.
- Team Player: Willingness to step into various roles to support colleagues as needed.
- Caregiver & Nursing Assessment: Expertise in evaluating caregiver and nursing skills and personalities to ensure a suitable match for each client's needs.
- Professionalism & Accountability: As the primary point of contact for clients, you must respond promptly to ensure they receive the necessary support.
- Availability: Willingness to work outside of normal business hours as needed for patient emergencies.
- Education:Bachelor's degree in healthcare or a related field preferred.
- Experience: Previous experience in customer service or healthcare, with a preference for roles involving client interaction or intake processes.
