Summary
The Community Liaison serves as the primary liaison to prospective patients, families, and designated healthcare providers, including: physicians, health care facilities, case managers, social workers, discharge planners, insurance case management professionals, and community groups. The liaison is actively involved in program development by one on one contact with referral sources.
Essential Job Duties AND Responsibilities
- Recruits referrals and coordinates with office staff on scheduling home health services.
- Obtains financial information as necessary from patient, family, and chart.
- Reviews chart and interviews patient and/or family to gather information to determine special needs.
- Begins patient/family education regarding home health, admission protocols, and financial information as needed.
- Is alert to changes in patient condition that would affect patient being admitted to home health services.
- If patient is medically inappropriate, Community Liaison will notify the referral source.
- Prospecting, marketing, and making presentations. Most job tasks will be outside of the office.
- Develops and maintains a thorough understanding of the territory assigned, target markets, and the business of existing and potential referral sources within assigned target market. Keeps abreast of general economic and business conditions as they relate to assigned territory, target markets, and referral sources.
- Schedules activities to identify new referral sources by making an average of five quality appointments per day, with an average of 20 contacts per week.
- Develops and maintains a thorough understanding of facilities' patient treatment programs, philosophies, goals, and objectives.
- Works with appropriate hospital staff members to provide service to existing accounts and consistently strives to increase the number of referrals from accounts.
- Develops meaningful business and personal rapport with key individuals within all customer accounts.
- Provides management with relevant, detailed information, regarding market place development and opportunities, including any accounts lost to the competition.
- Participates in appropriate professional, community organizations, and outreach activities.
- Maintains statistics and prepares/submits reports as required.
- Adheres to all hospital and corporate policies and procedures.
- Responds to patient and peer requests/needs in a timely manner.
- Accurately, clearly, and concisely records all pertinent information on patient medical records following established charting procedures.
