Fairview Nursing & Rehabilitation Center is seeking an experienced and compassionate Social Worker to join our dedicated healthcare team. The ideal candidate will have prior experience in a Skilled Nursing Facility (SNF) or Long-Term Care setting and possess strong knowledge of admissions and discharge planning.
Social Worker Summary
The Social Worker will assist residents and families with emotional, social, and transitional needs while ensuring a smooth admission and discharge process. This role requires excellent communication, organizational skills, and the ability to collaborate effectively with interdisciplinary healthcare teams.
Social Worker Responsibilities
- Coordinate and support resident admissions and discharge planning
- Provide emotional support and counseling to residents and families
- Assist with care planning and resident advocacy
- Coordinate community resources and post-discharge services
- Maintain accurate documentation in compliance with state and federal regulations
- Work closely with nursing staff, therapy, and administration to ensure quality resident care
- Address resident concerns and assist with conflict resolution when needed
- Previous experience in a Skilled Nursing Facility or Long-Term Care setting required
- Experience with admissions and discharge planning strongly preferred
- Bachelor’s degree in social work or related field preferred
- Strong communication and interpersonal skills
- Knowledge of healthcare regulations and resident rights
- Ability to multitask and work in a fast-paced environment
