Description:Care Coordinator Job DescriptionWe are currently seeking a Care Coordinator to join our team in an in-office role. This position is ideal for someone who is organized, compassionate, and experienced in VA, Private Pay, Long Term Care, or Medicaid home care scheduling and caregiver recruitment.Job TypeFull-timeIn-office (Not a remote position)ScheduleMonday – Friday8:00 AM – 4:30 PMThis is a position that will be performed in office. Remote or hybrid work schedules may be utilized depending upon the needs of Allpro Staffnet, LLCPay depending on experiencePosition SummaryThe Care Coordinator plays a key role in supporting our clients and caregivers by managing schedules, coordinating services, and recruiting caregivers to meet staffing needs. This position requires strong communication skills, attention to detail, and the ability to work independently in a fast-paced environment.Key ResponsibilitiesAccept new referrals and enter client demographics, service authorizations, and care needs into scheduling softwareDocument all client and caregiver interactions in the AxisCare systemContact new clients within 24 hours of referral and new staff within one weekCoordinate daily caregiver staffing to maximize authorized service hours and meet agency KPI metricsMaintain timely communication with the Operations Manager, Caregivers, VA staff, and Clients regarding schedule changes, critical incidents, or changes in conditionEnsure services align with care plans, urgent requests, and service agreementsRecruit, interview, and hire caregivers to support assigned caseload needsAttend job fairs and assist with local recruitment efforts as neededPerform other duties as assignedEVV & Visit ComplianceMaintain a minimum of 90% compliance score in each EVV system as required by MCO.Monitor Electronic Visit Verification (EVV) compliance through scheduling and visit oversight systems, including AxisCare and any state-mandated portalsReview and correct visit exceptions, failed visits, overlaps, missed clocks, and documentation issues daily.Assist caregivers with clock-in/clock-out troubleshooting and EVV compliance education.Verify visit information and escalate unresolved compliance concerns appropriately.Ensure manual visit confirmations and required documentation are completed timely.Maintain accurate service documentation and scheduling notes for audit readiness.Benefits of working for Allpro:Our full-time office employees are eligible for additional benefits, including the following:Health InsuranceDental InsuranceVision InsuranceRetirement PlanningCompany Paid Life InsuranceCompany Paid Long-Term DisabilityPaid Time OffSick TimeBonus OpportunitiesProfessional DevelopmentEmployee Discount ProgramAllpro is an equal opportunity employer and prohibits discrimination/harassment based on race, color, religion, age, sex, national origin, disability status, genetics, protected veteran status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state, or local laws.Requirements:QualificationsHigh School Diploma or GED requiredHomecare scheduling and recruiting experience requiredExperience with AxisCare or similar scheduling software requiredStrong organizational skills and attention to detailAbility to prioritize and manage multiple tasks efficientlyExcellent written and verbal communication skillsProficiency in Microsoft Office (Word, Excel, Teams)Ability to adapt to new tasks and changing prioritiesCompassionate, dependable, and a team-oriented self-starterPhysical RequirementsProlonged periods of sitting at a desk and working on a computerMust be able to lift up to 15 pounds#INDCORP
