This role involves assessing needs, developing care plans, providing crisis intervention, and linking clients to appropriate community resources. You’ll collaborate closely with MedNOW’s healthcare providers and administrative teams to ensure every patient receives integrated, patient-centered care that supports their overall well-being.CORE RESPONSIBILITIESAssess client needs: Evaluate each client’s situation, strengths, and support systems to establish goals and identify areas of need.Develop care plans: Design and implement individualized care plans to help clients achieve stability and success.Connect with resources: Refer clients to and advocate for access to community programs such as food assistance, childcare, housing, and healthcare services.Crisis intervention: Provide immediate support in urgent situations, including domestic violence, child welfare, or mental-health emergencies.Advocate for clients: Promote clients’ rights and ensure they receive equitable access to services and support.Monitor progress: Follow up with clients to evaluate progress and modify care plans as needed.Maintain records: Keep detailed, accurate, and confidential documentation of all interactions in accordance with MedNOW and HIPAA standards.Collaborate with others: Work in partnership with healthcare providers, schools, social service agencies, and community organizations to deliver coordinated care. QUALIFICATIONSBachelor Degree in Social Work (BSW) from a CSWE-accredited programMinimum of 1 year of professional experience in social work, case management, or behavioral health settingKnowledge of community resources and social service systemsStrong organizational, communication, and interpersonal skillsCPR certification requiredProficiency in electronic medical records (AthenaHealth EMR experience preferred)Why Join MedNOWAt MedNOW, you’ll find a workplace that values your compassion, expertise, and balance.
