Our goal is to help residents maintain dignity, independence, and the highest possible quality of life throughout their stay.We are a people-first organization guided by integrity, teamwork, and meaningful outcomes.Position SummaryThe LCSW oversees social service support for residents in our skilled nursing facility. This role works closely with residents, families, care teams, and community resources to ensure residents receive the support and advocacy they need.ResponsibilitiesAdvocate for resident rights and individual needsCommunicate with residents and families regarding care updates, goals, and discharge planningCoordinate referrals and services with outside agencies and community resourcesAssist residents with financial, legal, and supportive service resourcesBuild positive relationships between residents, families, and staffEducate team members on resident-centered care and psychosocial supportParticipate in interdisciplinary care plan meetings and develop appropriate plans of careAddress and assist with resident or family concerns and grievancesComplete social service assessments and discharge planning documentationMaintain accurate and timely documentation in the electronic medical record systemSupervise and support Social Services staff as neededRecreational Therapy ServicesMental Health ReferralsDevelops, implements and oversees behavioral programming and individualized interventions.
