Northern Manhattan Perinatal Partnership (NMPP) is a maternal and child health agency located in Central Harlem. NMPP's mission is to assist and support pregnant and parenting women in Central Harlem and Washington Heights neighborhoods to have healthy birth outcomes and empower women throughout their maternal cycles. NMPP provides case management services, advocacy, empowerment and supportive services to women in the identified communities.

NMPP seeks a Clinical Social Worker for its Preventing Early Adverse Childhood Experience (PEACE) for Moms Program. This initiative is a trauma -informed mental health project that provides group based and individual support to pregnant and postpartum women. The program an .eight session evidence informed parenting group, Mommy and Me bonding sessions, and brief supportive counseling to improve parenting self-efficacy, emotional well being and engagement in care. The project is a collaboration with the New York Presbyterian (NYP) Family PEACE Trauma Treatment Center (FPTTC).

The Clinical Social Worker will collaborate with the Community Health Workers (CHWs) . The CHWs will conduct initial screening for Adverse Childhood Experiences (ACE) and maternal depression.

Responsibilities

1. Facilitate an eight week PEACE for Moms support group for pregnant and postpartum women, throughout the year.

2. Conduct ongoing open-format Mommy and Me weekly support groups with interactive activities for mothers and their infant /toddlers

3. Provide short-term, individual supportive counseling, for a small caseload of pregnant or postpartum women

4. Refer clients with severe of persistent symptoms to external providers.

5. Maintain relationships with community service providers to support client referrals and collaboration

6. Enter and track participant data in the Well Family Data System, monitor and maintain accurate statistical data

7. Complete all required administrative tasks, including data quality checks, screenings

Qualifications

* Maters degree in Social Work ( LCSW or LMSW) or Mental Health Counseling (LHMC) with active New York licensure

* Minimum of three years experience working with high risk populations, preferably in maternal child health or community based mental health services

* Demonstrated experience facilitating groups

* Strong interpersonal and communication skills, culturally sensitive to a diverse population

* Able to be organized, flexible, and be able to manage tasks

* Ability to have practical solutions to problems

* Excellent writing skills and must be computer literate

Bilingual English/ Spanish or English/ French
