About the Role
As a Mental Health Therapist at ACP, you'll deliver high-quality psychotherapy and mental health services to adults experiencing diverse mental health challenges, including:
- Serious and Persistent Mental Illness (SPMI)
- Depression and anxiety
- Substance use disorders
- Cognitive decline and dementia
- Complex medical and psychosocial issues
Key Responsibilities:
- Conduct initial mental health assessments and develop individualized treatment plans.
- Provide evidence-based therapy and mental health interventions.
- Travel to various medical and community settings to reach clients who may have barriers to accessing traditional in-office care.
- Collaborate with physicians, nurses, social workers, and other healthcare professionals.
- Monitor clients' progress, adjusting treatment plans as needed.
- Maintain accurate and timely clinical documentation.

What We're Looking For
- Master's or Doctoral degree (PsyD or PhD) in Psychology, Social Work, Marriage and Family Therapy, or related field.
- Active Minnesota license as an LICSW, LP, LMFT, or LPCC
- Strong clinical skills and a passion for working with diverse and older populations.
- Commitment to culturally responsive care and integrated health models.
- Ability to work independently and collaboratively within multidisciplinary teams.
