JOB TITLE: Counselor
REPORTS TO: Clinical Director
DESCRIPTION: Oversee client care for assigned caseload. Conduct individual and group sessions, coordination of treatment planning including: assist with referral and collateral contacts, case management and completion of biopsychosocial assessment. Must have a working knowledge of ASAM levels of care and the DSM-5 criteria for substance use disorders.
DUTIES AND RESPONSIBILITY:
● Upon admission, obtain history and presenting information as well as orient new patient to program process and procedures.
● Provide patient with daily individual and group counseling, crisis intervention, as well as education and skills training.
● Collaboratively develops and updates an individualized treatment plan with the patient.
● Define all relevant community supports affecting treatment patient care including attorneys, family members, and other collateral agencies. Obtain signed releases of information to communicate with community supports.
● Update referring sources and family members on patient progress, if approved by the patient.
● Utilize patient-centered evidence-based interventions and rapport-building practices, including motivational interviewing techniques to enhance patient insight regarding diagnoses and patterns of use.
● Document treatment episodes, contacts, treatment plans and sessions according to regulatory compliance requirements.
● Coordinate comprehensive discharge plan to ensure seamless community reintegration.

QUALIFICATIONS:
Minimum Requirements
● Bachelor’s Degree and/or 2 years’ of addiction medicine or behavioral health experience serving culturally diverse populations.
Preferred Requirements
● Master’s Degree in human services, social work, psychology, mental health, addiction medicine or related field.
● LCSW, LMHC, LMFT, LPC, LADC I, LADC II preferred.

Job Type: Full-time
Pay: $45,000.00 - $60,000.00 per year
