- Salary Range: $24.04-$33.66 per hour, based on relevant years of experience
- Location: Loveland, Colorado (Hybrid: Orchards, Community-Based, & Remote)
- Status: Part-time, 20 hours per week
- Hours: ideally 8a-5p Wednesdays and Thursdays, 8a-12p Fridays

Role Overview:
The Encompass Care Coordinator works on a small, multidisciplinary team that provides intensive community-based services to children and families who have experienced trauma. The Care Coordinator provides direct support to families in navigating transitions in care, including through referral, intake, and stepdown into less intensive treatment. The Care Coordinator also provides case management services as needed and supports program tracking and data collection efforts. The ideal candidate for this position will be motivated and take initiative, have strong communication skills while interacting with clients, staff, and community partners, and be detail-oriented and willing to follow and develop essential workflows.
Essential Duties:
- Provide direct support to families in navigating their behavioral healthcare.
- Coordinate and communicate as needed with other SummitStone staff and departments, including intake, referrals, operations, and clinical teams.
- Provide case management to families experiencing challenges in accessing community resources.
- Provide program staff and clients with information about upcoming events, support groups, and community resources.
- Maintain relationships with community partners,including the Department of Human Services.
- Perform other duties as assigned.
Required:
- Bachelor’s degree in psychology, social work or related field from accredited College or University. Equivalent combinations of education and experience may be considered.
- 1-3 years minimum directly relevant experience in behavioral health field.
- Experience in coordinating services with other agencies.
Licenses and Certifications:
- Valid Colorado driver’s license or the ability to obtain one within 30 days of hire required; must maintain a clean driving record. This role requires travel to client homes and community partner sites.
