We are expanding from Southern California into Northern California – and we have a high volume of patients ready to be served right now.

Everest Peak Home Health is looking for experienced Registered Nurses (RN), Medical Social Workers (MSW), and Physical Therapists (PT) to join our NorCal team immediately. If you want a full caseload from day one, we have the referrals waiting.

Open Roles & Responsibilities

RN (Registered Nurse)

• Start of care, assessments, wound care, medication management
• Patient/family education and care coordination

MSW (Medical Social Worker)

• Psychosocial assessments, counseling, resource linkage
• Crisis intervention and advance care planning

PT (Physical Therapist)

• Mobility evaluations, fall prevention, therapeutic exercises
• Functional goal setting and home exercise programs

Requirements (by role)

• RN: Active California RN license + 1 year home health or acute care experience
• MSW: Active California MSW license + Master’s in Social Work + 1 year home health exp.
• PT: Active California PT license + Doctorate/Master’s in PT + 1 year home health exp.
• Valid driver’s license, reliable vehicle, auto insurance
• Current CPR/BLS certification

San Francisco, Oakland, San Jose, Sacramento, Fremont, Santa Rosa, Modesto, Stockton
