OCES supports older adults and individuals with disabilities by providing vital information and coordination of services. By promoting healthy, safe living for our consumers, we hope to allow them to stay active and engaged in their lives and in their own communities for as long as possible.

Our mission: Through the talents of an experienced and diverse workforce, OCES supports the independence and dignity of older adults and individuals with disabilities by providing essential information and services that promote healthy, safe living which positively impacts our community.

We are seeking an Options Counselor to provide pertinent information in response to service and eligibility inquiries; facilitates appropriate service provision and engages in activities related to the initial application process; maintains up-to-date information concerning resources and services available to applicants, consumers, and staff.

What you'll do:

  • Perform initial and ongoing assessments of the health and functional status of members
  • Develop community-based care plans and related service packages
  • Participate in the development of a member's Individual Care Plan (ICP)
  • Arrange and coordinate the provision of appropriate community long term care, social support services or other community-based services
  • Track member transfers from one setting to another and work with the PCT to adjust the ICP as appropriate
  • Promote the independent functioning of members and provide services in the most appropriate, least restrictive environment
  • Maintain records for each member in accordance with regulations and standards

What you'll bring:

  • Ability to respect the workplace and providers privacy and confidentiality
  • Strong analytical skills; attention to detail, accuracy, and due dates
  • Ability to organize and prioritize work with minimal supervision
  • Minimum of two years' experience in the care of persons 65 and older, with at least one year involving work in a setting involving health care services
  • Current Social Work license preferred
