Full Time Social Worker Transitional Housing Program

Job Description:

The Heart Matters Foster Family Agency is looking for a full-time Agency Social Worker (ASW) to carry a caseload of young adults 18-20 in our transitional housing program. The ASW conducts weekly visits, completes assessments and develops individual case plans and treatment objectives for each client to facilitate successful placement. The ASW will ensure home compliance and conduct trainings for the youth to help facilitate career/school enrollment. The ASW will consult with the Social Work Supervisor to ensure quality and ethically sound services that also maintain compliance with State, County and Agency regulations.

Qualifications:

• Master’s Degree in Social Work, Counseling or other related field recognized by Community Care Licensing
• Experience in working in child and adolescent services
• Knowledge of Community Care Licensing Regulations preferred
• Must have the ability to work effectively with others as a member of a treatment team
• Must obtain Fingerprint and Child Abuse Index clearances from the Department of Justice
• Must meet all requirements set by Community Care Licensing pertaining to the hiring of employees by a licensed community care facility
• Must be licensed to drive and insured in the state of California

Employee Status: Exempt
