Job Summary
We are seeking a passionate and proactive Lead Care Manager, to join our team in Southern California. The ideal candidate will be instrumental in recruiting patients eligible for ECM services—specifically those with high hospital or emergency department utilization, individuals being discharged from skilled nursing facilities, and those at risk of long-term institutionalization. This role involves being able to read and write care plans, patient engagement, and ongoing care coordination for up to one year per patient.
Qualifications
- Ability to read and write care plans
- Previous medical work experience desired; Medical Assistant etc.
- Exceptional organizational skills and follow thru
- Demonstrated ability to work effectively with community partners, healthcare professionals, patients, families
- Ability to work independently and collaboratively within a team
- Proficiency in basic computer applications (e.g., Microsoft Office Suite)
- Must possess a reliable vehicle and a valid driver's license. Willingness to travel locally within SoCal. Knowledge of local community resources and healthcare landscape is highly desirable
- What We Offer: Competitive hourly wage of $22 to $25/hour.
- Mileage reimbursement paid at standard IRS government rate
