The Social Services Director responsibility will be to provide for the physical, psychological, and social needs of the residents and all other duties assigned. Additionally, the Social Service Director shares a responsibility toward creating and sustaining an environment that humanizes and individualizes each resident. The Social Service Directors main responsibility will be to motivate all residents to make healthy adjustments to the nursing facility, by their participation in activities and social events. Informal counseling will be a part of the Social Service Director job responsibilities, and they will attempt to uncover any problems which might be interfering with the residents’ socialization and participation in home activities.
Additional Responsibilities Include:
- Responsible for all functions and responsibilities of the social services department
- Accountable for completion of social service assessments to include the MDS
Responsible for developing social service care plan and incorporate into the interdisciplinary care plan
- Serve on the interdisciplinary care team
- Initiate discharge planning services to include home health services, medical equipment, transportation services, and physician appointments
- Involvement in customer satisfaction and service recovery
- Able to meet firm deadlines for submission of Assessments, MDS and Care Plans
- Passionate about serving and caring for elders
