Position Summary:
The Service Coordinator supports participants in accessing and coordinating services through Medicaid Waiver programs, the MA State Plan, and other social and medical resources. This role manages an assigned caseload and ensures participant-centered planning, service coordination, compliance, and ongoing engagement through a hybrid work model combining remote administrative work and field-based participant support.
Position Scope:
The Service Coordinator is responsible for coordinating services, monitoring participant outcomes, and advocating for individuals across home and community-based settings. The position requires consistent participant interaction, collaboration with providers and agencies, and detailed documentation to ensure services align with participant goals, regulatory requirements, and funding authorizations.
Key Responsibilities:
This role ensures timely, compliant, and person-centered service coordination for assigned participants. The Service Coordinator independently manages caseload responsibilities while collaborating with interdisciplinary teams to promote participant well-being, independence, and continuity of care.
Secondary Duties:
- Assist in identifying alternative or supplemental funding sources
- Support quality assurance activities and internal reviews
- Participate in professional development and continuing education
- Represent PA Health Management in community and partner interactions as assigned
- Conduct person-centered assessments and level-of-care evaluations at least annually
- Develop, implement, and update individualized service plans at least quarterly or as needed
- Assist participants in identifying and accessing authorized waiver, medical, and social services
- Coordinate services with healthcare providers, community agencies, and government entities
- Monitor participant needs, outcomes, and service effectiveness
- Conduct required face-to-face participant visits and routine telephone contacts
- Advocate for participant needs and promote independence and quality of life
- Enter all required documentation accurately and within established timelines
- Participate in meetings, trainings, and interdisciplinary coordination activities
- Timeliness and accuracy of assessments and service plans
- Compliance with visit, contact, and documentation requirements

Maintains strict confidentiality of participant, family, and organizational information in accordance with HIPAA, privacy laws, and PA Health Management policies.
Documentation Requirements:
- Participant assessments and evaluations
- Individualized service plans and revisions
- Visit notes and service coordination records
- Compliance and regulatory documentation completed within required timeframes
- Bachelor’s degree in Social Work, Psychology, Human Services, or a related field required
- Equivalent combination of education and relevant experience may be considered
- Minimum of three (3) years of experience in service coordination, case management, or a related human services role preferred
- Experience working with waiver-funded or I/DD populations strongly preferred
Proficiency in Microsoft Office Suite and Microsoft Teams. Ability to work within multiple electronic documentation and data systems. Strong data entry, record-keeping, and administrative accuracy
Languages:
Fluency in English is required.
Driver License:
Valid driver’s license required. Reliable transportation required for regional travel.
Travel Requirement:
Regular travel required within a designated regional area. In-person attendance required for meetings, trainings, and team events as scheduled.
