Job Overview
We are seeking a compassionate and organized Resident Care Coordinator to join our dedicated team in providing exceptional care and support to residents in a senior living or assisted living community. In this vital role, you will serve as the primary point of contact for residents, families, and staff, ensuring personalized care plans are implemented effectively. Your energetic approach and attention to detail will foster a warm, safe, and engaging environment where residents thrive. This position offers an opportunity to make a meaningful difference in the lives of individuals with diverse needs, including those requiring Alzheimer's care, dementia support, or assistance with daily activities.

Duties

  • Develop, implement, and monitor individualized care plans tailored to each resident’s needs, including assistance with ADLs (Activities of Daily Living) such as bathing, dressing, and mobility.
  • Coordinate medical appointments, medication administration, and vital signs monitoring in compliance with HIPAA regulations and medical protocols.
  • Assist with resident activities like meal preparation, light cleaning, and engaging social or recreational activities to promote well-being.
  • Provide direct support during emergencies or behavioral challenges using conflict management and behavior management techniques.
  • Maintain accurate documentation within EMR (Electronic Medical Record) systems, including care plans, incident reports, and progress notes.
  • Conduct regular assessments of residents’ health status and communicate updates to nursing staff and family members.
  • Support residents’ emotional well-being through compassionate interaction and personalized attention while adhering to safety standards such as first aid protocols.

Requirements

  • Prior experience working in assisted living facilities, nursing homes, or home care environments is highly preferred.
  • Knowledge of Alzheimer’s care, memory care strategies, dementia care, and working with individuals with disabilities or developmental challenges.
  • Strong understanding of medical terminology, medication administration procedures, and HIPAA compliance.
  • Ability to perform physical tasks including heavy lifting and the use of assistive devices like Hoyer lifts or resident’s .
  • Valid driver’s license with a clean driving record for transportation duties if required.
  • Excellent communication skills for collaborating with team members, residents, families, and healthcare providers.
  • Certifications such as CPR/First Aid are a plus; ongoing training in hospice care or autism support is advantageous. Join us in creating a caring community where every resident feels valued!
