Overview
We are seeking a dedicated and dynamic Master Social Worker to join our healthcare team, providing expert clinical support across diverse settings. In this vital role, you will deliver comprehensive social work services to patients and their families, ensuring holistic care that addresses emotional, psychological, and social needs.   • Conduct thorough patient assessments and reassessments, including diagnostic evaluations and mental health screenings, to determine individual needs and develop personalized care plans.
  • Coordinate intake procedures for new patients and manage ongoing case management activities to ensure seamless service delivery.
  • Collaborate with multidisciplinary teams to develop and implement treatment plans that incorporate medical documentation, HIPAA compliance, ICD coding (International Classification of Diseases), CPT coding (Current Procedural Terminology), and detailed medical records management.
  • Offer crisis intervention and crisis management support to stabilize patients during acute episodes or emergencies.
  • Facilitate home & community care program referrals , assisted living support, hospice care, and inpatient services to promote recovery in various settings.
  • Maintain accurate documentation of patient care activities using Electronic Medical Record (EMR) systems while adhering to strict confidentiality standards under HIPAA regulations.
  • Work with older adults, seniors, individuals with disabilities or developmental challenges such as Alzheimer’s disease or dementia; provide specialized services tailored to each population’s unique needs.
  • Familiarity with medical terminology, CPT coding procedures, and medical records management is essential.
  • Demonstrated ability to perform comprehensive patient assessments.
  • Knowledge of crisis intervention techniques and experience managing complex cases involving dementia care will set candidates apart.
  • Excellent communication skills for engaging patients during intake sessions and providing individual/group counseling effectively.
