Job Summary:

The Outreach Care Coordinator serves as a key connection between community health outreach efforts and patient care services. This role is responsible for following up with individuals who attend community health events, educating prospective patients on the importance of vascular and podiatry care, and coordinating appointments with providers.

The ideal candidate is highly personable, organized, goal-oriented, and comfortable both scheduling appointments and communicating the value of preventative and specialized care services. This position also requires accurate tracking and reporting of outreach and patient conversion data.


Key Responsibilities:

  • Follow up with individuals who attend community health events through phone calls, text messages, and other communication methods.
  • Educate prospective patients on vascular and podiatry conditions, available treatment options, and the importance of early intervention and preventative care.
  • Schedule new patient consultations and appointments for vascular and podiatry providers.
  • Build rapport and maintain professional communication with prospective patients to encourage engagement in care.
  • Support outreach initiatives by helping convert community interactions into scheduled patient visits.
  • Maintain accurate documentation of outreach activities, patient interactions, scheduling outcomes, and follow-up attempts.
  • Track and report outreach event data, including leads generated, appointments scheduled, patient conversions, and related metrics.
  • Coordinate with marketing, outreach, and clinical teams to ensure smooth patient onboarding and continuity of communication.
  • Assist with identifying opportunities to improve outreach follow-up processes and patient engagement strategies.
  • Represent the organization professionally and compassionately in all patient interactions.

Qualifications

  • High school diploma or equivalent required; associate degree or healthcare-related experience preferred.
  • Previous experience in healthcare scheduling, patient coordination, outreach, sales, customer service, or medical office operations preferred.
  • Strong communication and interpersonal skills.
  • Comfortable discussing healthcare services and encouraging prospective patients to seek care.
  • Ability to multitask, prioritize, and maintain detailed records.
  • Proficiency with electronic medical records (EMR), scheduling systems, spreadsheets, and basic reporting tools.
  • Experience with community outreach or healthcare marketing is a plus.
  • Bilingual skills are a plus.
