Care Plan Coordinator

Job Overview:

MRSC Solutions LLC is seeking a compassionate and organized Care Plan Coordinator to support patients receiving virtual psychiatric and mental health services. This role is responsible for coordinating individualized treatment plans, assisting patients throughout their care journey, and ensuring smooth communication between providers, patients, pharmacies, and insurance representatives.

The ideal candidate is detail-oriented, empathetic, and experienced in behavioral health or healthcare coordination. This position plays a key role in helping patients manage conditions such as anxiety, depression, ADHD, PTSD, bipolar disorder, insomnia, and other mental health concerns through structured telehealth support.

Responsibilities:

• Coordinate individualized patient care plans and follow-up schedules
• Communicate with psychiatric providers regarding treatment updates and patient needs
• Assist patients with appointment scheduling and telehealth onboarding
• Monitor patient progress and document updates in electronic health records (EHR)
• Verify insurance information and assist with referrals or authorizations
• Support medication management coordination and pharmacy communication
• Educate patients on treatment plans, resources, and next steps
• Maintain HIPAA compliance and patient confidentiality standards
• Collaborate with therapists, psychiatric nurse practitioners, and support staff
• Ensure continuity of care through regular patient outreach and follow-ups

Qualifications:

• Previous experience in healthcare coordination, behavioral health, case management, or mental health services preferred
• Strong communication and organizational skills
• Knowledge of medical terminology and behavioral health practices
• Experience with EHR/EMR systems
• Ability to work independently in a remote healthcare environment
• Understanding of HIPAA regulations and patient confidentiality
• Associate’s or Bachelor’s degree in Healthcare Administration, Psychology, Social Work, Nursing, or related field preferred

Pay: $24.00 - $27.80 per hour
