ABOUT THE ROLE:

SUMMARY:

Support Mobile Crisis Assessors with client follow-up and coordination of services. This role will work closely with clients and families to connect them with appropriate community resources and services.

SUPERVISORY RESPONSIBILITIES:

ESSENTIAL DUTIES AND RESPONSIBILITIES:

The Duties and Responsibilities reflect the general details considered necessary to describe the principal functions of the job and shall not be construed as a detailed description of all related work assignments that may be inherent to the job.

  • Provide coordination of care, directly to the person and the family across transitions of care, including in agency services and community resources.
  • Will work as a member of an interdisciplinary Mobile Crisis Response Team and other professionals to develop a plan for recipients and/or make revisions to existing plan components as appropriate.
  • Complete all documentation in a timely manner according to department policies and procedures. This includes mandated data tracking elements.
  • Monitor the presenting mental illness and/or co-occurring symptoms.
  • Help maintain the self-respect, personal dignity and physical safety of each recipient.
  • Work cooperatively with all other staff and relate favorably to recipient and families.
  • Attend all mandatory staff meetings and consumer conferences or give advance notice to direct Supervisor as to reasons for inability to attend.
  • Receive supervision by direct Supervisor.
  • Report any changes in consumer’s condition, family situation, or needs to the appropriate personnel.
  • Report to direct Supervisor and/or Designee any suspected abuse, neglect or exploitation of recipients.
  • Able to drive to locations within the Program service area.
  • Be familiar with PPBH policies and procedures including the securing of emergency services.
  • Be familiar with HIPAA regulations and abide by them.
  • Obtain and keep current all training required by the agency.
  • Perform any other duties as required by Supervisor.

CORE COMPETENCIES:

  • Maturity and sensitivity to the special needs of PPBH clients while possessing the knowledge of behavioral health.
  • Ability to communicate client needs and information to behavioral healthcare staff.
  • Ability to utilize and navigate an electronic health record and Microsoft office software.
  • Must be service oriented with good customer service skills.
  • Demonstrated ability to adhere to a flexible schedule which allows for evening and weekend hours as may be required to respond to individual needs.
  • Must possess the ability to maintain client confidentiality.

QUALIFICATIONS, EDUCATION AND/OR EXPEREINCE:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

  • Bachelor’s degree in a mental health or related field with at least 3 years of case mgt. experience.
  • Ability to communicate effectively, both verbally and in writing.
  • Knowledge of community resources including functions of other social service agencies desirable.
  • Excellent organizational skills.
  • Ability to prioritize the needs of the clients.
  • Able to work independently.
  • Skill in providing support to individuals on a short term bases.
  • Ability to develop and maintain effective professional work relationships and therapeutic relationships with individuals.
  • COMPUTER SKILLS: Must have Intermediate to advanced experience in MS Office Suite (Outlook, Word, Excel, PowerPoint), the use of personal computers, and standard office productivity software.
  • REASONING ABILITY: Ability to apply commons sense understanding to carry our instructions furnished in written, oral or diagram form.

REQUIREMENTS:

  • A valid Class E Drivers’ License for positions that require driving personal or Agency vehicle on Agency business.
  • Must maintain a minimum Bodily Injury & Property Liability insurance limit of at least $50,000 for positions that require driving personal own vehicle (POV) on Agency business. (Proof Required)
  • PPBH is a 24/7 Agency. All employees must be available as needed when required.
