Daily Pay
Pay Rate: $18.00-$20.00/hour
Responsibilities:
- Regularly assess and interact with patients and families to ensure individualized care needs are met
- Attend Medicare meetings, subacute meetings, departmental meetings, and other assigned meetings
- Provide therapeutic interactions and counseling to patients and families
- Complete all documentation timely and accurately
- Participate in interdisciplinary care plan meetings and invite patients and families to attend
- Assist with transportation and setup for activities
- Inform residents of various activities available to them
- Maintain confidentiality and adhere to Department of Health regulations
Physical Requirements:
- Push a wheelchair
- Lift a medical record
- Be able to visit each assigned resident/patient within their domain
- Visual acuity is necessary to read medical charts and observe residents
Qualifications:
- Bachelor's degree in a human service field preferred
- Previous case management and documentation experience preferred
- Previous experience in healthcare required
- Interview and therapeutic relationship skills
- Knowledge of community, Human service agencies, and medical terminology
