Pay Rate: $18.00-$20.00/hour

Responsibilities:

  • Regularly assess and interact with patients and families to ensure individualized care needs are met
  • Attend Medicare meetings, subacute meetings, departmental meetings, and other assigned meetings
  • Provide therapeutic interactions and counseling to patients and families
  • Complete all documentation timely and accurately
  • Participate in interdisciplinary care plan meetings and invite patients and families to attend
  • Assist with transportation and setup for activities
  • Inform residents of various activities available to them
  • Maintain confidentiality and adhere to Department of Health regulations

Physical Requirements:

  • Push a wheelchair
  • Lift a medical record
  • Be able to visit each assigned resident/patient within their domain
  • Visual acuity is necessary to read medical charts and observe residents

Qualifications:

  • Bachelor's degree in a human service field preferred
  • Previous case management and documentation experience preferred
  • Previous experience in healthcare required
  • Interview and therapeutic relationship skills
  • Knowledge of community, Human service agencies, and medical terminology
