Job Summary:

The Clinic Care Coordinator supports patients with chronic conditions and complex needs by providing care coordination, patient education, and resource navigation under the direction of the provider. This role identifies patients who would benefit from care management, monitors adherence to care plans, assists with overcoming barriers to care, and connects patients to internal and community resources. The Clinic Care Coordinator plays a key role in improving patient outcomes, continuity of care, and overall patient experience.

Other information:

Bachelors degree in nursing, social work or other related clinical field preferred. Three to four years experience preferred. Ambulatory and/or care management experience preferred. Current Drivers License in good standing and reliable and insured transportation

This job will be authorized 80.00 hours bi-weekly.
