Proposed Schedule: 40 hours per week. Monday-Friday 8am-4:30pm.
The Homeward Bound/Palliative Care Coordinator is responsible for administrative support and coordination for the Homeward Bound and Palliative Care Services, with a primary focus on scheduling and coordinating home visits for providers serving patients in the community. This position requires flexibility due to providing home-based primary care to patients in the community. This position is a vital part of the Interdisciplinary Team, coordinating schedules to meet the needs of the patients, families and other team members.
- Coordinate provider schedules for home visits and/or outpatient palliative care services
- Schedule patients as directed by provider or patient needs using mapping tools to schedule by geographical location to ensure efficiency in provider's travel
- Arrange referrals to medical specialists, Public Health and other agencies for patients as indicated by the other health care team members
- Review electronic medical records prior to patient visits and enter required documentation from other sources to assist team with pre-visit planning
- Prepare any paper documents needed for home visits and/or outpatient Palliative Care appointments to assist Homeward Bound/Palliative Care team members
- Forwards calls, faxes or other incoming correspondence to appropriate team members, creating a patient case or physically talking with team members as indicated
- Complete check-in/check-out, collect co-pays, co-insurance and self-pay balances at time of visit
- Accurately enter and verify patient demographics and insurance information/coverage over the phone and/or at check-in
- Protect the confidentiality of patient's electronic medical record in accordance with policy and procedure
- Assist providers and team with all calls and fax transmissions
- Assist with maintaining various program tracking tools for program and reporting
- Participate in quality improvement activities for the program
- Participate in Interdisciplinary Team meetings and weekly case review
- Help coordinate transitions of care with fellow Homeward Bound team members from hospital, Emergency Room, Rehab/Nursing Home setting and any other area of transitions
- Provides highest level of customer service in all interactions with internal and external customers
- Classifies and barcodes hard copy documents for electronic medical records
- Reviews and follows policies and legal requirements prior to releasing copies of medical records
- Other duties as assigned
- High School diploma required
- 3-5 years Medical Office Assistant experience preferred
- Well organized with demonstrated ability to manage multiple tasks and prioritize workload
- Proficient computer competencies including Microsoft applications and Electronic Medical Record experience preferred
- Knowledge of billing a plus
- Ability to work independently, as a team member, and with patients/caregivers/families through verbal and written communication providing excellent customer service
- Must have excellent telephone skills, be able to manage challenging conversations, and maintain calm under pressure
The pay rate for this position is $19.77/hour.
