Social Services Director MSW

We are seeking a Social Services Director (LSW) to lead the social services program in our skilled nursing facility. This role supports residents and families through admissions, care planning, and discharge planning while ensuring compliance with state and federal regulations.

Qualifications

• Licensed Social Worker (LSW) – Florida
• Experience in skilled nursing, long-term care, or rehabilitation preferred
• Strong knowledge of discharge planning and community resources
• Excellent communication and documentation skills
• Experience with EMR/EHR systems preferred

Responsibilities

• Complete psychosocial assessments for residents
• Coordinate discharge planning and transitions of care
• Participate in care plan meetings with the interdisciplinary team
• Provide support and guidance to residents and families
• Maintain accurate EHR documentation
• Address resident concerns and assist with grievances
