With our residents in mind, we are seeking an experienced Social Worker for our Hillsborough location. Schedule is as follows: Monday-Friday 8:30AM-4:30PM

Duties/Responsibilities:

• Develop relationships and collaborate with the primary care teams to conduct social services assessments to determine the appropriate needs for each resident
• Assess psycho/social well-being of patients.
• Coordinate and participate in Care Plan Meetings
• Acting as a liaison with social, health and community agencies
• Discharge Planning
• Develop relationships with physicians and families

Qualifications/ Skills:

• Must be able to read, write and communicate in English
• Experience in long term care preferred
• Experience with screening, assessment, and planning for common social services needs
• Strong interpersonal communication skills with exceptional active listening abilities
• Highly empathetic, non-judgmental, and open-minded
• Experience in a collaborative team environment

Education and Experience:

• Certified or Licensed Social Worker in the State of NJ
• Bachelors Degree (Required) in Social Work or Psychology. Master’s Degree preferred
• 1+ years' experience in clinical social work
• A problem-solving orientation and a flexible and positive attitude

• Proficient PC skills
