A well-established, mission-driven non-profit senior care organization is seeking a compassionate and organized Social Worker to support its Skilled Nursing/Rehab population within a beautiful Continuing Care Retirement Community.

This role works closely with the Director of Social Services and interdisciplinary team to support residents and families through care planning, psychosocial support, discharge coordination, and communication throughout the continuum of care.

Position Highlights:

• Monday-Friday, 7:30a-4p

Ideal Background:

• Bachelor’s Degree in Psychology, Sociology, Social Work, or related field required
• Minimum 3 years of Long-Term Care LSW experience in PA preferred
• Strong communication, organization, and documentation skills
• Knowledge of community resources and resident advocacy
