  • Document all patient interactions accurately within electronic medical records (EMR) systems while adhering to HIPAA regulations for confidentiality and data security.
  • Coordinate case management activities including patient care planning, community resource linkage, and follow-up to promote optimal health outcomes.
  • Collaborate with multidisciplinary teams—including medical professionals, educators in special education settings, and community organizations—to support comprehensive patient care.

  • Familiarity with EMR systems for efficient documentation and data collection; experience with clinical research is a plus.
  • Ability to handle complex cases involving PTSD care, addiction counseling, Geriatrics, or working with individuals with disabilities across diverse settings including hospital environments or home & community care contexts.
  • Excellent crisis intervention skills combined with compassionate social work practices that prioritize patient safety and empowerment.
  • Knowledge of HIPAA compliance standards along with strong organizational skills for managing medical records and patient assessments effectively. Join us in delivering impactful telehealth services that transform lives through accessible mental health support!
