About the Role
As a Care Coordinator, you’ll serve as a lifeline and advocate for our elderly and disabled members, helping them receive the services they need to remain safely in their homes. You’ll be assigned a caseload and will visit members every 90 days, maintain regular phone contact, and keep their care plans up to date based on their evolving needs. This is a hybrid role requiring local travel and monthly in-office presence (Atlanta location).

Key Responsibilities

  • Conduct in-home visits every 90 days and monthly follow-ups via phone/text/email
  • Build strong relationships with clients and caregivers to assess needs and services.
  • Monitor care delivery to ensure services meet client needs effectively
  • Coordinate services and collaborate with providers, family members, and medical professionals
  • Maintain up-to-date, confidential case documentation
  • Assist in arranging emergency or enhanced community-based services as needed
  • Participate in case conferences with nurses (RNs/LPNs) to ensure care plans align
  • Identify and advocate for non-traditional services that enhance client well-being
  • Maintain consistent communication with your supervisor and participate in team trainings
  • Report any signs of suspected abuse, neglect, or exploitation
  • Travel within your assigned territory to serve members and attend weekly team meetings at the office

What You Bring

  • Bachelor’s degree in Social Work, Sociology, Psychology, or a related field
  • Strong communication, organization, and time management skills
  • A heart for helping others, especially vulnerable populations
  • Ability to work independently and problem-solve in real time
  • Comfort with technology and case documentation tools
  • Reliable transportation and willingness to travel locally

Preferred but Not Required

  • Experience with Georgia’s CCSP/EDWP Waiver Programs
  • Background in case management, healthcare, or home health
